We read with interest the recent and important articles by Kechele et al. and Bertelli et al. in which they presented the alleviated tension sutures technique using an external epineurial splint to overcome a 7-mm nerve gap in the median nerve of rats and a mean of 28-mm nerve gap in the median nerve of six patients. In 1987, we described a similar method for repairing nerves in five patients; the epiperineurial stitches used were 6-0 to 8-0 according to the size of the nerve under repair and were secured, instead that at a mesh, directly to the surrounding fascia tissue.
