A 40-year-old alcoholic man with a past medical history ofhypertension presented to the emergency department withprogressive fatigue and dyspnea on exertion for about3 months. Physical examination was remarkable only for paleconjunctiva. He had pancytopenia (white blood cells0.5 9 109/L, hemoglobin 6.3 g/dL and platelets 31 9 109/L)and a normal coagulation panel. A peripheral blood smearshowed pancytopenia with unremarkable morphology of thefew existing circulating white cells. Other routine laboratorytests were within normal limits. With a presumptive diagnosisof alcohol-induced bone marrow suppression, he receivedfilgrastim injections (300 mcg daily) for 3 days until hisWBCcount reached 10.4 9 109/L. A peripheral blood smear at thistime revealed 63 % immature cells, many of which containedAuer rods.
