Scrotal pouch as a urinary reservoir: a surgical revolution or bizarre hypothesis?

I read this article Verit et al. with great interest. However there are some facts limiting this hypothesis. First, this procedure is not applicable for women due to the use of scrotal pouch. Second, bilateral orchiectomy or luteinizing hormone releasing hormone agonists represent the standard of care for metastatic prostate cancer not for the localized cancers . In the article also cited by Verit et al., significant prostate cancer is detected in 25 of 121 cystoprostatectomy specimens without any lymph node metastasis . This means there was no indication for bilateral orchiectomy in that present bladder cancer population. The other recommended option, fixation of testis to the psoas muscle will increase the risk of testicular cancer and may lead to testicular atrophy as a cause of increased temperature .
