Historically, boiler tube failure (BTF) is one of the leading causes of forced outages of fossil fuel power generating stations. It is also the number one equipment problem in industrial systems. This article discusses the evaluation of a boiler tube failure at a 40-year-old, 200-psig O-type package boiler at an oil refinery, including the past failure history, roles of mechanical and water chemistry issues, and remedial measures taken to alleviate such failures.
