Combination prophylaxis for thromboembolism prevention among gynecologic oncology patients perioperatively

We appreciate the Letters to the Editor by Dr Wallach and Dr Durfee that highlight areas of controversy regarding thromboembolism prophylaxis. There are limited data providing guidance in preventive management of gynecologic oncology patients perioperatively, and therefore there is room for controversy.Dr Wallach was "shocked to read suggestions of a 'randomized trials' to evaluate VTE prophylaxis". We would clarify that we would never consider using a "placebo only" arm to any future clinical trial, because the benefits of single modality prophylaxis appear to be well established in gynecologic oncology.
