The definitive cause of adrenal androgen excess in polycystic ovary syndrome (PCOS) remains an enigma despite the efforts and passion of several investigators over a 20-year period . from 1979 to 1999. The article by Azziz et al. (1) resurrects this focus of investigation that has been dormant for more than a decade and highlights some of the concepts and clinical research techniques involved in addressing this issue in earlier studies.
