Every otolaryngologist, including myself, has invariably faced an ethical dilemma that has led to both personal and professional anguish. Whether it involved a request to perform a tracheotomy of uncertain benefit on a critically ill woman, a profoundly hearing-impaired child whose parents were reticent to proceed with co-chlear implantation, or a frail man with a borderline-resectable tumor, as surgeons we collectively struggle to know what is "right" at these critical moments.
