New opportunities? Community nursing and the five year forward view

The report identifies several 'emerging' models of care— seven in total—and says NHS England will identify which of these will suit specific 'health communities'. Indeed, it is proposed that in some places several of these models will 'evolve in parallel'. Much of the debate concerns two specific models (see Table i),both of which have significant implications for community nursing.
