The Patient Centered Medical Home (PCMH) is an approach to providing comprehensive primary care that enhances effective communications between patients and their physicians. The PCMH is being implemented in healthcare organisations in the USA with enthusiasm and endorsement by policymakers and organisations responsible for reimbursement Team-based care is a critical feature of the PCMH model.
