Basic surgical training (BST) posts are very oversubscribed, both by British and overseas doctors. The changes to be implemented as part of Modernising Medical Careers will make entry into surgical training more controlled. This will streamline training for British graduates but may make it harder for overseas doctors to compete. There are proposals to allow more experienced Trust doctors to enter a surgical programme at a more senior level if they can demonstrate appropriate competencies. This may be particularly applicable to overseas graduates too experienced for foundation posts. Our study examines the surgical experience of doctors applying for a BST rotation, particularly that of the overseas applicants.
