Dear Editor, We report an interesting case of a high order pregnancy that developed after a transfer of two frozen/thawed embryos resulted in a monozygotic octuplet anembryomolar pregnancy. A 34-year-old woman and her reproductive partner were referred to the Reproductive Medicine Center for infertility. A comprehensive examination revealed that the female suffered from bilateral fallopian tube occlusion and endometriosis (diagnosed by laparoscopy), and the male presented with oligoasthenozoospermia. No multiple pregnancies have been acknowledged in the couple's family history. The ICSI procedure was recommended to the couple as a treatment for infertility. The ovarian stimulation was performed using the recombinant follicle stimulating hormone (150 IU) and human menopausal gonadotrophin (225 IU). Gonadotrophin-releasing hormone antagonist (0.25 mg) was used to prevent LH signalling. The duration of induction was 9 days. Human chorionic gonadotrophin (5000 IU) was given to trigger ovulation 36 h prior to ovarian retrieval. Twelve oocytes were recovered: five in metaphase II, five in germinal vesicle breakdown and two germinal vesicle. Ten oocytes were fertilised. Seven oocytes advanced to the two pronuclei stage after ICSI. Two of these pronuclear pre-embryos were frozen, and five underwent cultivation. On the post-fertilisation day 3, two embryos were transferred into the uterus. This fertilisation cycle was unsuccessful.
