We read with interest the article of Yoshida et al. in which the authors indicate there are correlations between ionized and total magnesium levels during administration of magnesium sulfate in patients with preterm labor and preeclampsia. The authors conclude that it is desirable to measure the level of ionized magnesium, the physiologic active form in vivo. However, if ionized measurements are not possible, the authors suggest that total magnesium values may be converted to ionized magnesium levels. In contrast, we previously demonstrated no significant correlation between ionized and total magnesium levels in patients receiving magnesium for preterm labor and/or preeclampsia .
