Pancreaticocolonic fistula (PCF) is a rare sequela of necrotizing pancreatitis. Typically, surgical intervention is required because spontaneous closure is infrequent and persistent infection can be catastrophic. In this video, we demonstrated the presence of 2 PCFs by instillation of a mixture of radiopaque contrast material and methylene blue through a transgastric endoscope placed in the necrosectomy cavity, with simultaneous fluoroscopic and colonoscopic views of the fistula sites in the descending colon.
