Massive hepatic portal venous gas and gastric pneumatosis secondary to gastric ischemia

A 68-year-old diabetic woman with a history of coronary artery disease, ischemic left-sided cardiomyopathy, and peripheral vascular disease presented with 1 day of sudden abdominal pain and coffee-ground emesis. Her vital signs were normal, and her abdomen was mildly tender without peritoneal signs. A CT of the abdomen showed massive hepatic portal vein gas and gastric pneumatosis (A).
