We read with interest the new technique for Zenker's diverticulum (ZD) septotomy proposed by Ramchandani and Nageshwar Reddy,1 in which a new scissor-shaped SB diathermic knife (Sumitomo Bakelite, Tokyo, Japan), a monopolar rotating device with insulated claw-curved blades, developed for endoscopic submucosal dissection (ESD) was used. The authors successfully performed 1 diverticuioscope-assisted and 2 cap-assisted septotomies, concluding that the SB knife is safer, easier to use than standard needle-knives, and may obviate the use of a diverticuloscope.
