Mantle-cell lymphoma.

An 84-year-old woman with a 6-month history of general weakness, fatigue, decrease in appetite, 18.1 kg (40 lb) weight loss, and two episodes of melena 1 week before presentation, presented with 10 episodes of small, tarry stool mixed with red blood for 1 day. Physical examination revealed a 60.3 kg (133 lb), chronically ill appearing, elderly woman. There was no palpable peripheral lymphadenopathy.The abdomen was soft, nontender, nondistended, with normal active bowel sound and without evidence of palpable mass or hepatosplenomegaly. Peripheral bloodshowed white count 25.7 k/mm~3 (normal: 4-10 k/mm~3) with normal differential; Hb 8.1 g/DL (12-16 g/DL), hematocrit 24.3% (35-48%). Flow cytometry showed CD19, 84.6% (3-13%);CD20, 837% (6-12%); CD5, 79% (59-72%); and CD23, 0.4%.
