Endoscopic creation of a gastrogastric conduit for reversal of gastric bypass.

Gastric bypass surgery poses various challenges to the endoscopist because of the surgically altered anatomy.1 Reconnecting the gastric pouch to the excluded stomach for the reversal of gastric bypass may be required in special circumstances, and surgical intervention is often the only option available.Simultaneous antegrade and retrograde endoscopy has been reported for complete esophageal obstruction2"4 and with EUS for colonic obstruction.5 The antegrade endo-scope is inserted via the mouth, and the retrograde endo-scope is inserted via the G-tube tract to establish luminal patency. A similar approach was used for the reversal of gastric bypass by using 2 simultaneous endoscopes.
