Balloon sweep in portal biliopathy.

Sharma and Ponnusamy1 recently reported their experience with bleeding from choledochal varices in patients with choledocholithiasis and coexistent portal biliopathy and suggest that removal of calculi may be safer with a Dormia basket than with balloons. However, because in each of their 3 cases both baskets and balloons were used, to say that only balloon sweeping is responsible for hemobilia is questionable. Such a recommendation could only be made if balloon sweeping were the sole modality used.
