Transient paraplegia because of epidural anaesthesia in a post op THR patient!

A 60-year-old obese male with COPD presented to us with a history of pain and swelling around the left hip following a history of fall at home. He had undergone a hemiarthroplasty with an uncemented bipolar prosthesis about 3 years prior to this injury. X-ray revealed a type B2 (Vancouver classification) periprosthetic fracture for which we decided to do a total hip replacement (THR) with a long stem (Fig. 1). The patient underwent a pre anaesthetic check-up and was cleared for surgery. It was decided to operate on the patient under combined spinal epidural anaesthesia to give good post operative pain relief.
