It is considered that RT actually preserves sexual function to a greater extent than nerve sparing RP conducted in selected centres of excellence due to several factors. Patients treated with RT techniques are always several years older in comparison to patients before RP and as a result comorbidities are more common. Both age and comorbidity are strongly predictive for the preservation of sexual function after both treatment methods [ 1 -3 ] and lead to an imbalance against RT. A pre-selection of patients with excellent pre-treatment sexual function for nerve sparing RP has the same consequence. In contrast to RP, RT techniques are - at least currently - performed without particularly discriminating patients for sexual function preservation. In addition, with increased positive margin rates after nerve sparing RP, oncologic results are not potentially compromised after RT.
