Words of wisdom. Re: Perineal anastomotic urethroplasty for posttraumatic urethral stricture with or without previous urethral manipulations: a review of 61 cases with long-term followup.

The authors retrospectively reviewed the outcome in patients who underwent bulboprostatic anastomosis for strictures following pelvic trauma. Sixty-one patients (mean age: 34 yr) were included in this study, and the impact of prior urethral manipulation in the final outcome was assessed. The mean follow-up was 67 mo. The surgical technique is fully described, including the use of ancillary maneuvers to obtain a tension-free anastomosis. The gap between bulbar urethra and prostatic apex was 1-7 cm in length. Out of procedures performed in 61 patients, 52 (85.2%) were considered successful and 9 (14.8%) were considered failures. The failure rate was higher in patients who underwent prior urethral manipulation (19% vs 12.5%). The authors concluded that prior urethral manipulation may influence the outcome of posterior urethroplasty.
