Re: Complications of Porcine Small Intestinal Submucosa (SIS) Graft for Peyronie's disease (PD)

In this retrospective review, the authors reported their unfavorable experience with one-layer small intestinal submucosa (SIS) graft in 19 patients with Peyronie's disease (PD) with a mean follow-up of 15 mo. Of these patients, 18 (95%) had primarily dorsal curvature and 5 (26%) had concurrent hour-glass deformity. The authors noted no difference in Sexual Health Inventory for Men (SHIM) scores before and after surgery. Preoperatively, 12 men (63%) had erectile dysfunction (ED), whereas 10 (53%) reported postoperative ED. More importantly, 7 patients (37%) had recurrent penile curvature, 5 (26%) had recurrent Peyronie's plaque, and 12 (63%) had penile shortening. In addition, the rate of complications such as hematoma and graft infection was as high as 37%. Thus, the authors concluded that, compared with saphenous vein, SIS is not a better graft material because of its high recurrence rate and complications.
