Priapism is a persistent, unwanted erection that is not associated with sexual desire or sexual stimulation. There are three types of priapism; ischemic priapism, nonischemic priapism, and recurrent (stuttering) priapism. Ischemic priapism is a urologic emergency and has the potential to cause erectile dysfunction through ischemic injury to cavernous tissue; it necessitates early evaluation and treatment. If medical management is unsuccessful in achieving detumescence, a surgical approach is necessary. The aim of the surgery is to allow blood to flow in and out of the corpora cavernosa again by creating a new route for venous return. A distal cavernoglanular (corporoglanular) shunt should be the first choice, given its ease to perform and its association with few complications.
