Uni- or bilateral antegrade cerebral perfusion: That is the question!

First, I want to congratulate Urbanski et al. [1] for the excellent results they have achieved in aortic surgery with the technique of arterial cannulation of the right carotid artery, unilateral cerebral perfusion and hypothermic circulatory arrest at a blood temperature of 28°C.In the manuscript, the effectiveness of unilateral cerebral perfusion (UCP) combined with moderate hypothermic circulatory arrest was evaluated in a cohort of 122 patients with a mean duration of UCP of 38 ± 18 min. Patients with a circulatory arrest time <20 min were excluded from the study (257 patients). 36% of patients were operated on for acute type A aortic dissection and 73% underwent total or subtotal arch replacement. The fact that nearly two-thirds of aortic patients had an UCP duration <20 min and that the mean duration of the UCP time of the remainder exhibit a mean UCP time of 38 min underlines the excellence and routine practice of Urbanski's group in aortic surgery.
