Pityriasis rubra pilaris with prominent acantholytic dyskeratosis.

A 68-year-old Caucasian man presented to our institution with a two month history of a diffuse scaling erythematous eruption involving the trunk, limbs and cephalic extremity. The eruption began with erythematous facial plaques and scaling of the scalp extending secondarily in a cephalocau-dal way to the trunk and the limbs, with an admixture of eczematiform and psoriasiform lesions, leading to a generalized erythroderma. Some islands of normal skin were present within the erythematous scaly plaques (figure 1A). An orange-coloured palmoplantar keratoderma progressively appeared. The patient complained of moderate pruritus. The clinical examination was otherwise normal. Past medical history included only childhood asthma. No family history of skin disease was found. No medication was being taken at the onset of the eruption.
