Balancing economic, ethical and equity concerns in orphan drugs and rare diseases

There is a significant policy debate regarding whether licensed treatments for very rare conditions should be paid for because of their high cost. Here we critically review the arguments for paying these premium prices and propose alternative strategies for the provision of treatments for rare diseases.
