Although the exact dose to achieve glycemic goal with once-daily basal insulin in type 2 diabetes is derived only by careful titration, knowing the average range for the dose can be helpful to the clinician. From data in a 2007 published review of basal insulin clinical trials (1), I was able to ascertain the association between the final mean fasting plasma glucose to the final mean dosage. I selected only those studies in which no bolus insulin was used and excluded insulin detemir trials because this insulin may have been given twice-daily with the total daily dose skewed by additional morning basal insulin given for prandial hyperglycemia following breakfast and lunch.
