Insulinoma in a Patient with Type 2 Diabetes Mellitus Proved at Autopsy

The coincidence of diabetes and insulinomas is exceedingly rare. Of 313 confirmed cases of insulinomas observed in a Mayo Clinic (Rochester, Minnesota) study, only 1 case of insulinoma occurring in a patient with diabetes was reported (1). The possible relationship between diabetes mellitus and the natural history of insulinomas is unknown. A 75-year-old man was referred to our hospital with a decreased level of consciousness. Although no detailed medical records were available, he declared that he had been diagnosed with type 2 diabetes mellitus and that he was prescribed some orally administered hypoglycemic agents and self-injected insulin.
