INTRODUCTION It is unfortunate that, just as has been observed with type 2 diabetes mellitus (T2DM), there has been a significant increase in the prevalence of type 1 diabetes mellitus (T1DM) throughout the world in the past decade. The International Diabetes Foundation reported that there will be more than one-half billion persons with diabetes by 2030, largely in emerging economies. China, India, and the United States will continue to lead the world with the largest numbers of persons with diabetes. Most persons with diabetes are between the ages of 40 and 59 years (1). A newly created registry is providing data for the first comprehensive analysis of patients with T1DM in the United States; the registry is collecting common data about living with and managing diabetes, as well as clinical outcomes that are being achieved in everyday clinical practice. The National Registry, funded by the Leona M. and Harry B. Helmsely Charitable Trust, includes approximately 25 000 patients of all ages and represents an ethnically diverse population.
