Case Presentation: A 59-year-old previously healthy man presented with 6 months of progressive vision loss, which was greater in the left than right eye and had become worse over the 2 weeks prior to presentation. He denied focal weakness, slurred speech, seizures, or headaches. On exam, he was alert and oriented with no focal weakness but had bilateral inferior-temporal visual field defects and pale optic nerves. Brain magnetic resonance imaging (MRI) revealed a suprasellar heterogeneous mass (5.3 x 2.7 x 2.5 cm) with solid and fluid components and displacement of the optic chiasm. The pituitary gland appeared normal (Fig. 1). Laboratory panel revealed a free thyroxine level of 0.8 ng/dL (normal, 0.5 to 1.6 ng/dL), thyroid-stimulating hormone level of 0.65 mlU/L (normal, 0.34 to 5.6 mlU/L), testosterone level of 250 ng/dL (normal, 270 to 1,100 ng/dL), follicle-stimulating hormone level of 6 IU/L (normal, 1 to 8 IU/L), luteinizing hormone level of 4 IU/L (normal, 2 to 12 IU/L), prolactin level of 2 ng/mL (normal, 2 to 19 ng/mL), insulin-like growth factor-1 level of 164 ng/dL (normal, 56 to 271 ng/dL), and a morning cortisol level of <0.4 mug/dL (normal, 6.7 to 22.6 mug/dL). The patient was on intravenous dexamethasone (4 mg) every 6 hours. Additional preoperative studies included computed tomography (CT) angiogram and cerebral angiogram (Fig. 2). What is the diagnosis?
