A novel technique for endoscopic snare polypectomy using a duodenoscope in combination with a colonoscope for the inaccessible colonic polyp.

Some polyps are located so inaccessibly in the colon that they prove impossible to remove using a colonoscope alone. This case report describes a novel technique for managing the really difficult polypectomy entirely endoscopically, using a duodenoscope in combination with a colonoscope. After insertion of the colonoscope to the level of the polyp, an overtube was inserted and the colonoscope withdrawn. A duodenoscope was then inserted through the overtube and the polyp was identified and snared. This procedure may make purely endoscopic polypectomy possible in patients in whom endoscopy would otherwise have had to be combined with laparoscopy or laparotomy and hence general anesthesia.
