Isolated Nonrespiratory Alternans in Doppler Tissue Imaging Velocities in a Patient with Ischemic Heart Disease

A 45-year-old, nonsmoker male presented with history of angina on effort. He was not taking any medicine. Pulse rate was 66/min. Blood pressure was 130/84 mmHg in right upper limb in supine position. Cardiovascular systems examination did not reveal any abnormality. Resting electrocardiograph was within normal limits. During treadmill stress test, there was ST segment depression in leads II, III, AVF, and V4 to V6, with complaint of angina (Fig. 1). Changes gradually normalized during recovery. Echocardiographic examination was performed just after treadmill test.
