The Petroleum Safety Authority Norway (PSA) has criticised ConocoPhillips following the collision between the vessel Big Orange XVIII and the Ekofisk 2/4-W facility in June. An investigation carried out by the PSA showed rules were broken and that people could have been killed in the accident. The vessel was on autopilot en route to the 2/4-X facility on the Ekofisk field to carry out well stimulation when the accident happened.
