Even with a positive cough stress test, the clinician still needs to know whether a particular patient is likely to be cured with a midurethral sling. A simple clinical test, performance of a "simulated" or "virtual" operation provides the answer: pressing upwards on the vagina with a forefinger or haemostat on one side immediately behind the pubic bone (see video in electronic supplementary material, ESM) will almost invariably control urine loss [2].
