Thromboelastography assessment of coagulation status in parturients with lupus anticoagulant receiving heparin therapy.

Lupus anticoagulant is an immunoglobulin that interferes with in vitro phospholipid-dependent coagulant tests such that there is laboratory prolongation of activated partial thromboplastin time (aPTT).However, circulating lupus anticoagulant is associated with arterial and venous thrombosis and such parturients with lupus anticoagulant may present to the labor suite taking heparin and requesting regional anesthesia. In these circumstances, conventional aPTT cannot be used to assess the presence of circulating free heparin. We used Thrombelastography (TEG~R) to assess the levels of circulating heparin before regional anesthesia in two parturients.
