The integrity of BP’s Macondo Well in the Gulf of Mexico began to gradually unravel around five hours before a fatal blowout claimed 11 lives on Apr. 20 and triggered a massive oil spill. Faulty cement work led to the chain reac- tion of failures, followed by complications with the blowout preventer (BOP) and mon- itoring systems. This was compounded by several oversights on the part of BP, Transocean and Halliburton employees aboard the Deepwater Horizon platform.
