Introduction: Otogenic brain abscess still is one of the most dangerous complications of otitis media with high risk of mortality. This condition typically involves a perforation of tympanic membrane, with intermittent or continuous otorrhea. Methods: Clinical report. Result: We present a case of a young man who presented to us with persistent unilateral otalgia for 1 year duration but with normal otoscopic ear examination. CT scan revealed erosion of ossicles and mastoid region with complicated left temporal lobe abscess. Conclusion: In a patient presented with chronic earache, it is important to rule out middle ear problem even when the patient does not have ear discharge. ? 2013 Japan International Cultural Exchange Foundation & Japan Health Sciences University.
