Ligamentum flavum haematoma (LFH) is rare, usually presenting after minor trauma with back pain and lower limb ataxia. We provide a previously unreported case of spontaneous LFH at the thoraco-lumbar junction. A 72-year-old female with a history of operated breast cancer, presented with a ten year history of episodic low back pain, an eight month period of typical lower limb neurogenic claudication and subjective sensory blunting in the left lower limb. There was no history of trauma or urinary symptoms. On examination there was no spinal tenderness, neurologic deficit in the lower limbs, nor root tension signs.
