The findings a decade ago that more people die each year from medical errors in US hospitals than from traffic accidents, breast cancer, or AIDS1 came as a surprise, even to experts in the emerging fields of health care quality and patient safety. We have made much progress in building a foundation to address patient safety since the publication of the Institute of Medicine's (IOM) report, To Err is Human: Building a Safer Health System, but substantial work remains to ensure that patients are safe in every setting where they receive health care.
