Editorial: Dementia and delirium - the need to fix the system.

This special issue brings together a range of papers that highlight issues associated with dementia and delirium. Reflecting on 'where we are at' with dementia services and the care of people with delirium made me realise that whilst we often 'bunch' these complex conditions together, in reality they are very different things and advances in each have taken very different journeys. Services for people living with a dementia have advanced over recent years. In most western societies, the idea that older people who are living with a dementia should be shut away from communities in large institutions has been largely eroded.
