Atherosclerotic double right coronary artery and ectasia of left coronary arteries in a patient with presented acute coronary syndrome and ventricular tachycardia.

Coronary artery abnormalities are not uncommon. Incidence varies between 0.2% and 2% in angiographic series, but atherosclerotic double right coronary artery (RCA) has been reported in only one case [1-10].To the best of our knowledge, this unusual variance of the coronary anomaly in association with ventricular tachycardia and ectasias of left coronary system has not been reported previously.A 56-year-old man applied to the emergency department with complaints of palpitation, syncope and chest pain, like angina. The patient declared that he fell down two times and stayed unconscious for 3-5 min 1 week before his application and on admission day. The patient suffered from typical chest pain on exertion for 1 year. He had hypertension and he smokes. The blood pressure and pulse rate were 140/90 mm Hg and 74 bpm, respectively. The physical examination was completely normal. There was T-wave inversion and rS pattern in II, III and aVF leads and 0.5-1 mm ST segment depression in V4-6 on electrocardiogram. A previous electrocardiogram revealed sustained monomorphic ventricular tachycardia. Cardiac enzymes and troponin-I was found to be increased (Troponin-I: 7.7ng/dl).
