We are writing to inform of a recent licensing change allowing tamsulosin to be available without prescription. Intraoperative floppy iris syndrome, the intraoperative effects of alpha-adrenergic blockers during cataract surgery, was first described in 2005 with tamsulosin but later found to be an alpha-blocker class effect. While strategies can be employed to counteract intraoperative difficulties, they do not eradicate the problem completely. alpha-Blocker use has been shown to lead to more complications during surgery, including a 1.5-fold increase in posterior capsular rupture rate (doxazosin).3 Frustratingly, stopping the offending drugs many weeks before surgery has no, or only limited, benefit, and some patients exhibit irreversibly reduced pupillary tone.
