Issuing guidance is only part of improving practice; subsequent implementation needs to be supported and monitored. Guidance, plans and reports are often intimidating - a daunting pile of information which is difficult to use and interpret. Too many reports gather dust and remain unread or unimplemented. When I read a report, I want to see accurate, reliable and synoptic information. Sadly, guidance often comes as 'read-only' information, with little involvement from the nurse. Until now that is.
