A 42-year-old man with a history of parathyroid carcinoma underwent total body scintigraphy and SPECT using Tc-99m MIBI for the evaluation of metastases. At the time of imaging, laboratory tests showed elevated levels of calcium and parathyroid hormone. Unexpected intense uptake was observed in the left maxillary sinus. Biopsy and histologic examination of the specimen revealed a brown tumor. The authors conclude that Tc-99m MIBI seems to be of limited value in staging parathyroid carcinoma, especially in patients with elevated parathyroid hormone levels.
