Patient safety: lessons have still not been learnt.

A 26-year-old nurse has been arrested on suspicion of three counts of murder at Stepping Hospital in Greater Manchester. Two further deaths are being investigated as suspicious. While the nurse has not yet been charged or convicted (at the time of going to press), this is a worrying turn of events which must serve to remind us of the gravity of patient safety. When people seek help from health or social care organizations, they quite rightly do not expect to experience less effective treatment or to suffer harm or undesirable consequences as a result of the organization's failure to learn.
