Managed care is struggling to deliver quantifiable value and, as a result, healthcare reform has been slow to take effect. Disease management, however, with its focus on measuring and reporting the outcomes of care across a defined population is gaining attention as a viable strategy to thwart the increasing costs of healthcare delivery while maintaining, if not improving the quality of care being delivered. In order to achieve these results, however, providers and payers will need to institutionalize the use of automated tools that systematically collect, provide decision support and report clinically and financially meaningful data. This systems-based approach will be critical to bringing about the necessary behavior change to impact both quality and cost as well as reform healthcare as we know it today.
