Multinodular goitre: an unusual cause in a patient with acromegaly

We present the study of a 60-year-old man with a 10-year history of acromegaly. He initially underwent trans-sphenoidal hypophysectomy, but his insulin-like growth factor-1 (IGF-1) remained elevated after surgery. He declined radiotherapy and was commenced on octreotide. This lowered, but did not normalize his IGF-1 level. Ten months after his diagnosis of acromegaly, an abdominal ultrasound showed a 9-cm renal cell carcinoma (RCC) in the right kidney. He underwent right nephrectomy and radiological studies in the next 5 years showed no evidence of recurrence.
