Iron deficiency anaemia: a presentation of idiopathic pulmonary haemosiderosis.

An 18-year-old man was referred for investigation of anaemia. He was well, taking no regular medications. He smoked 10 cigarettes/day for 3 years, consumed 10-20 g alcohol/day,occasional marijuana, but denied other drug usage. He had no HIV risk factors, recent travel or wildlife contact.He had 2 months pallor, lethargy, and 1 month of cough, with yellow sputum with occasional flecks of blood. He was breathless walking 30 m. He had no symptoms of gastrointestinal blood loss, weight loss, fevers or sweats. He had adequate iron intake.On examination, his blood pressure was 110/70, heart rate was 85 b.p.m. and oxygen saturation was 97% (room air). He was pale, with no lymphadenopathy, skin rashes or joint synovitis. Fine crepitations were audible bibasally. Cardiovascular examination was unremarkable.
