A 64-year-old man presented with a 1-week history of progressive ascending muscular weakness, which was now preventing him from standing or walking. He had mild myalgia of his thighs and calves and had noted a decrease in grip strength as well. He had no shortness of breath, subjective sensory impairment or fever. In the 2 weeks before this he had had watery diarrhoea approximately 10 times per day and also had persistent mild nausea. He had no abdominal pain, vomiting, melaena or haematochezia.
