Involved/uninvolved immunoglobulin ratio identifies monoclonal gammopathy of undetermined significance patients at high risk of progression to multiple myeloma

Monoclonal gammopathy of undetermined significance (MGUS) is a common asymptomatic plasma cell dyscrasia. A small number of MGUS cases progress to multiple myeloma (MM) and their early identification is of great importance. Different risk factors have been described (Kumar et al, 2005; Rajkumar et al, 2005; Perez-Persona et al,.. 2010). Recently, suppression of the uninvolved chain of the monoclonal immunoglobulin (Koulieris et al, 2012; Katzmann et al, 2013; Wolff et al, 2013) was proposed to play a role in stratifying the risk of progression. We aimed to validate these results and to further explore the role of heavy/light chain (HLC) pair quantification in identifying MGUS patients at risk of progression.
